About the Role

What You’ll Do

• Audit clinical QA performance by reviewing charts for accuracy, completeness, and consistency.
• Validate documentation support for services, identify incorrect classifications, and investigate automation errors.
• Partner with engineering and product teams to improve AI evaluation of clinical documentation.
• Identify missed clinical nuances, recommend decision rules, and validate improvements before deployment.
• Develop SOPs for reviewing home health and hospice documentation.
• Standardize quality expectations across agency workflows and payer requirements.
• Support customer implementations by understanding agency documentation and clinical workflows.
• Help configure automations for each customer’s operational processes.
• Track documentation quality, automation accuracy, exception rates, clinical escalations, and rework.

Required Qualifications

• Active Registered Nurse license is required; a New York or compact license is preferred.
• At least three years of home health and/or hospice clinical experience.
• Strong understanding of OASIS documentation and home health eligibility requirements.
• Experience reviewing physician orders, certifications, plans of care, face-to-face documentation, and hospice election paperwork.
• Hands-on experience with HomeCare HomeBase, Axxess, and WellSky; KanTime is a plus.
• Strong understanding of Medicare home health and hospice documentation requirements.
• Experience writing SOPs, QA guidelines, or training documentation.
• Excellent attention to detail, pattern recognition, and comfort in a fast-paced startup environment.

Nice to Have

• QA, Clinical Documentation Integrity, or compliance experience.
• Experience with AI, workflow automation, or healthcare technology companies.
• Familiarity with Careport and Forcura referral platforms.
• Experience implementing or optimizing EMR workflows.
