Responsibilities
- Deliver direct nursing care in accordance with individualized care plans and clinical protocols.
- Monitor and document patients' physical condition, emotional well being, behavior, and response to treatment.
- Administer prescribed medications and treatments safely and accurately within authorized scope.
- Observe patients for changes in condition and communicate concerns promptly to supervising RN or physician.
- Build therapeutic relationships with patients through compassionate care and effective communication.
- Assist patients with activities of daily living including personal hygiene, mobility, nutrition, and daily routines.
- Support patients in achieving individualized care goals through encouragement and education.
- Assist with admissions, transfers, discharges, and patient orientation.
- Maintain accurate, timely, and confidential clinical documentation using EMR system.
- Collaborate effectively with RNs, physicians, and other healthcare professionals.
- Respond appropriately to emergencies and assist in implementing safety procedures.

Overview
- The Practical Nurse (PN) is responsible for providing safe, compassionate, and patient centered nursing care under the supervision of Registered Nurses and physicians.
Key Responsibilities
- Deliver direct nursing care in accordance with individualized care plans and clinical protocols.
- Monitor and document patients' physical condition, emotional well being, behavior, and response to treatment, promptly reporting significant changes.
- Administer prescribed medications and treatments safely and accurately within the authorized scope of practice.
- Observe patients for changes in condition and communicate concerns promptly to the supervising Registered Nurse or physician.
- Build therapeutic relationships with patients through compassionate care, active listening, and effective communication.
- Assist patients with activities of daily living, including personal hygiene, mobility, nutrition, and maintaining daily routines.
- Support patients in achieving individualized care goals through encouragement, education, and ongoing assistance.
- Assist with admissions, transfers, discharges, and patient orientation.
- Maintain accurate, timely, and confidential clinical documentation using the hospital's electronic medical record (EMR) system.
- Collaborate effectively with Registered Nurses, physicians, and other healthcare professionals to ensure continuity and quality of care.
- Respond appropriately to emergencies and assist in implementing safety and crisis management procedures when required.
- Maintain patient dignity, confidentiality, and professional ethical standards at all times.
