Coordinates and handles care for patients receiving outpatient treatment.
Assesses patient needs, coordinates appointments, provides education and support, and promotes continuity of care.
Collaborates with healthcare providers, patients, and families throughout the treatment process.
Responsibilities
- Provides direct patient care, including administering medications, monitoring vital signs, and assisting with treatments.
- Conducts comprehensive assessments covering functional, environmental, psychosocial, and health information for discharge planning.
- Coordinates care plans and treatment schedules with healthcare professionals.
- Educates patients and families about diagnoses, treatment options, side effects, and coping with treatment challenges.
- Maintains accurate medical records and documents care, treatments, outcomes, and communications in the EMR.
- Provides counselling and emotional support and participates in patient-related meetings.
- Advocates for patient needs and ensures access to appropriate care and resources.
- Participates in clinical research trials and stays current with gastroenterology care advancements.
- Contributes to quality improvement, departmental goal setting, and evaluation of approaches to meeting goals.
- Monitors symptoms and treatments such as pain, nausea, and fatigue.
- Provides patients and relatives with a tour of the clinic.
- Applies infection-control and pain-management knowledge and performs additional duties as requested.
Qualifications
- Bachelor's degree in nursing from a minimum three-year full-time course.
- Valid license or registration to practice in the home country or country of last employment, where applicable.
- Valid Basic Life Support certification or Advanced Cardiac Life Support certification.
- A postgraduate degree in nurse management or an advanced clinical specialty is preferred.
Professional Experience
- Minimum five years of recent post-graduate experience in the related specialty.
- Ability to provide care and services based on patients' age, appropriateness, and developmental needs.
- Skills in assessing patients' and families' physical and psychological needs.
- Skills in coordinating, planning, and prioritizing care based on individual needs.
