- Coordinate developmental assessment appointments and therapy services
- Act as the primary liaison between families, healthcare providers, therapists, schools, and community agencies

- Develop, implement, and monitor individualized service plans
- Schedule appointments and maintain service calendars
- Ensure timely referrals to specialists and community support programs
- Maintain accurate and confidential client records and documentation
- Monitor service delivery and follow up on missed appointments or unmet needs
- Educate families about available services, benefits, and community resources
- Assist families with accessing financial assistance or government support programs when applicable
- Participate in multidisciplinary team meetings and case conferences
- Prepare reports, statistics, and documentation for quality assurance and compliance
Requirements
- Bachelor's degree in Social Work, Psychology, Child Development, Nursing, Healthcare Administration, Education, or a related field
- Relevant experience (typically 2–5 years) in child development, healthcare coordination, rehabilitation, or social services
- Knowledge of child development milestones and developmental disorders
- Familiarity with multidisciplinary healthcare or rehabilitation services
