Overview

  • Works independently, and is held responsible legally for failure to exercise due care in the dispensation of pharmaceuticals. Incumbent may perform on shift and is expected to be on call for emergencies as required.
  • Prepares medications by reviewing and interpreting physician orders detecting therapeutic incompatibilities.
  • Dispenses medications by compounding, packaging, and labeling pharmaceuticals.
  • Controls medications by dispensing according to DOH requirements.
  • Provides pharmacological information by answering questions and requests of health care professionals; counseling patients on drug therapies.
  • Protects patients and technicians by adhering to infection-control protocols.
  • Dispenses medications to adults and neonates after checking and counseling.
  • Participates in daily patient care rounds (NICU).
  • Facilitates appropriate ordering and preparation of Total Parenteral Nutrition (TPN) by identifying and resolving inappropriate orders
  • Calls physicians to clarify a prescription when in doubt.
  • Facilitates discharge/transfer for adults and neonatal patients:
  •  Anticipates discharge/transfer date and medications.
  •  Counsel/triage patients/caregivers on discharge/transfer medications as necessary.
  •  Participates in and facilitates care by parent’s orders.
  •  Facilitates discharge/transfer planning and smooth transition to alternate site of care or the outpatient pharmacy.
  •  Assesses insurance drug coverage well in advance of discharge/transfer and assists in preauthorization for medications.
  • Compounds medications and supervises its preparations.
  • Checks prescriptions before it is dispensed.
  • Dispenses and registers controlled and compounded prescriptions on their special books.
  • Keeps custody of controlled medication and abidance with its laws and regulations.
  • Spontaneous dispensing of some medications.
  • Advises Physicians on equivalents or substitute drugs.
  • Helps reviews drug usage patterns and suggests control when needed.
  • Ensures the safety and security of patients and staff when it comes to medication use. Bears responsibility for ongoing oversight of medication supplies to facilitate early detection of adulteration, abuse, or diversion.

  • Successfully completed and holds an accredited recognized BSc. in Pharmacy.
  • Holds a current and valid license to practice as a pharmacist in home country.
  • Must be strong and qualified clinical pharmacist with inpatient neonatal/pediatric experience.
  • Must have DOH License to practice or passing exam certificate
