Job Overview

  • Fixed term post until 13.03.2027 to cover secondment leave.
  • Seeking Band 5 Community Nurses to join District Nursing Team in Tywyn.
  • Opportunities to develop into Band 6 District Nurse, Chronic Condition Case Manager, or Nurse Practitioner.

Main duties of the job

  • Provide clinically effective care within the District Nurse Team.
  • Deliver high quality evidence based care to maintain patients in their own homes.
  • Avert hospital admissions and empower patients towards self management.
  • Implement seamless patient centred care and treatment.

Person specification Qualifications

  • Essential: Registered nurse or within 2 months of gaining PIN.
  • Essential: Knowledge of Community Care.
  • Essential: Practice supervisor/assessor.
  • Desirable: Previous community experience.

  • Essential: Effective interpersonal and communication skills.
  • Essential: Ability to maintain high standards of care.
  • Essential: Clinical competency.
  • Essential: Competent in catheter care, PICC line care, wound care, leg ulcer care, palliative care, CRT working.
  • Desirable: Flexible approach to work.
