• **Counseling & Emotional Support:** Provide guidance to patients and families dealing with stress, trauma, chronic illness, or end-of-life situations.
• **Discharge Coordination:** Arrange post-hospital care such as rehabilitation, home healthcare, or placement in long-term care facilities.
• **Resource Connection:** Link patients with community resources, financial aid programs, housing services, or support groups.
• **Advocacy:** Ensure patients’ rights are respected and help them navigate healthcare systems and insurance processes.
• **Crisis Intervention:** Assist in emergency situations like abuse cases, mental health crises, or sudden illness.
