Job Overview
Responsible for dispensing and reviewing medications, ensuring medication safety, supporting physicians and nurses with drug-related information, monitoring patients' medication therapy, and maintaining compliance with hospital policies and regulatory requirements.
Key Responsibilities
- Review and verify physician medication orders and prescriptions.
- Dispense inpatient and outpatient medications accurately.
- Conduct medication reconciliation and identify medication discrepancies.
- Monitor drug therapy for efficacy, safety, interactions, contraindications, and adverse drug reactions.
- Provide clinical pharmacy support to physicians, nurses, and other healthcare professionals.
- Counsel patients regarding medication use, dosage, administration, precautions, and side effects.
- Participate in multidisciplinary healthcare teams and patient-care rounds.
- Maintain accurate medication profiles and pharmacy documentation.
- Manage controlled medications and maintain appropriate records.
- Monitor medication storage, inventory, expiry dates, and stock levels.
- Support antimicrobial stewardship and rational drug use.
- Assist with formulary management and medication-use evaluations.
Qualifications
- Bachelor’s degree in Pharmacy (B.Pharm) or Doctor of Pharmacy (Pharm.D).
- Valid DHA pharmacist license/registration.
- Knowledge of pharmacology, therapeutics, pharmaceutical calculations, medication safety, and drug interactions.
- Knowledge of hospital pharmacy policies, regulatory requirements, and infection-control procedures.
- Good communication, clinical reasoning, documentation, and computer skills.
- 3-6 years of hospital/clinical pharmacy experience, depending on the position.
- Experience reviewing and validating medication orders and prescriptions.
- Experience with inpatient and outpatient medication dispensing.
- Experience managing controlled medications and pharmacy inventory.
