Job Overview

Responsible for dispensing and reviewing medications, ensuring medication safety, supporting physicians and nurses with drug-related information, monitoring patients' medication therapy, and maintaining compliance with hospital policies and regulatory requirements.

Key Responsibilities

  • Review and verify physician medication orders and prescriptions.
  • Dispense inpatient and outpatient medications accurately.
  • Conduct medication reconciliation and identify medication discrepancies.
  • Monitor drug therapy for efficacy, safety, interactions, contraindications, and adverse drug reactions.
  • Provide clinical pharmacy support to physicians, nurses, and other healthcare professionals.
  • Counsel patients regarding medication use, dosage, administration, precautions, and side effects.
  • Participate in multidisciplinary healthcare teams and patient-care rounds.
  • Maintain accurate medication profiles and pharmacy documentation.
  • Manage controlled medications and maintain appropriate records.
  • Monitor medication storage, inventory, expiry dates, and stock levels.
  • Support antimicrobial stewardship and rational drug use.
  • Assist with formulary management and medication-use evaluations.

Qualifications

  • Bachelor’s degree in Pharmacy (B.Pharm) or Doctor of Pharmacy (Pharm.D).
  • Valid DHA pharmacist license/registration.
  • Knowledge of pharmacology, therapeutics, pharmaceutical calculations, medication safety, and drug interactions.
  • Knowledge of hospital pharmacy policies, regulatory requirements, and infection-control procedures.
  • Good communication, clinical reasoning, documentation, and computer skills.
  • 3-6 years of hospital/clinical pharmacy experience, depending on the position.
  • Experience reviewing and validating medication orders and prescriptions.
  • Experience with inpatient and outpatient medication dispensing.
  • Experience managing controlled medications and pharmacy inventory.
