SSBZH is seeking a compassionate and clinically skilled
Registered Nurse - Long-Term Acute Care
to provide high‑quality, patient‑centred care to individuals with complex medical conditions requiring extended hospitalization.
The role focuses on delivering comprehensive nursing care to patients with chronic illnesses, post‑acute needs, and long‑term recovery requirements.
The role will work closely with physicians and multidisciplinary teams to manage complex care plans, monitor patient progress, and support recovery in alignment with Department of Health (DOH) regulations and organizational standards.
Responsibilities
- Deliver comprehensive nursing care to LTAC patients with complex, chronic, and post‑acute conditions, following evidence‑based practice.
- Conduct thorough patient assessments, monitor vital signs, clinical status, and response to treatment, and escalate changes appropriately.
- Administer medications, IV therapies, enteral/parenteral nutrition, and other treatments in accordance with physician orders and nursing protocols.
- Manage patients requiring long‑term ventilation support, wound care, tracheostomy care, and complex medical interventions (as applicable).
- Collaborate with physicians, respiratory therapists, rehabilitation teams, and case managers to support individualized care plans and discharge planning.
- Maintain accurate and timely clinical documentation using electronic medical record (EMR) systems.
- Educate patients and families on disease management, treatment plans, and ongoing care needs to support recovery and transition of care.
- Adhere to infection prevention, patient safety, and quality standards, participating in audits and continuous improvement initiatives.
Qualifications
- Bachelor’s Degree in Nursing (or equivalent recognized qualification)
- Valid DOH/DHA/MOH Registered Nurse license (or DOH eligibility)
- Minimum 2-3 years of clinical nursing experience, preferably in LTAC, medical‑surgical, ICU, or step‑down units
- Strong assessment, critical thinking, and clinical decision‑making skills
- Experience managing patients with complex or long‑term medical needs is an advantage
- BLS certification required; ACLS preferred
- Willingness to work rotating shifts, as required
