The Doctor is responsible for delivering chronic condition follow-up services to targeted population, aligned with departmental objectives, while ensuring high-quality care, patient safety, and an enhanced member experience
Responsibilities & Duties

- Manage chronic conditions through appropriate lab requests, interpretation, and medication refills.
