Job Summary
Join the Chandler Care Center team as a Physical Therapy Assistant and make a meaningful impact in the lives of our residents. As a compassionate and motivated individual, you'll play a vital role in our commitment to providing exceptional, state-of-the-art care. As a valued member of our team, you'll have the opportunity to work closely with our physical therapists to develop and implement individualized treatment plans that promote optimal health and well-being.
Responsibilities
- Assist physical therapists in the development and implementation of treatment plans for residents
- Conduct physical therapy treatments under the supervision of the physical therapist, including exercises, activities, and other interventions
- Monitor resident progress and report any changes or concerns to the physical therapist
- Maintain accurate records of resident treatment plans, including progress notes, treatment plans, and discharge summaries
- Participate in interdisciplinary team meetings to discuss resident care and goals
- Collaborate with other healthcare professionals to provide holistic care that meets the unique needs of each resident
