Responsibilities
- Review, interpret, and validate outpatient prescriptions to ensure accuracy and appropriateness
- Accurately prepare, label, dispense, and deliver medications in compliance with DOH regulations
- Provide patient counselling on medication usage, dosage, potential side effects, and adherence
- Maintain and update patient medication records and profiles in a timely manner
- Monitor and manage medication inventory, including handling controlled substances and expiry tracking
- Ensure compliance with infection control, patient safety protocols, and quality standards
- Collaborate with physicians and healthcare teams to support safe prescribing practices
Requirements
- Bachelor's degree in Pharmacy
- Relevant clinical exposure, internship, or placement in a hospital and/or retail pharmacy setting
- Department of Health (DOH) license holder
- Knowledge of key pharmaceutical procedures and current best practices
- Good understanding of pharmacy related regulations
- Practical knowledge in Microsoft Office suite

Responsibilities
- Review, interpret, and validate outpatient prescriptions to ensure accuracy, appropriateness, and identification of any therapeutic incompatibilities
- Accurately prepare, label, dispense, and deliver medications in compliance with Department of Health (DOH) regulations and internal pharmacy standards
- Provide patient counselling on medication usage, dosage, potential side effects, and adherence to ensure safe and effective treatment outcomes
- Maintain and update patient medication records and profiles in a timely and accurate manner
- Monitor and manage medication inventory, including handling controlled substances, expiry tracking, and removal of damaged or outdated stock
- Ensure compliance with infection control, patient safety protocols, and quality standards within the outpatient pharmacy setting
- Collaborate with physicians and healthcare teams to support safe prescribing practices and contribute to clinical and patient care initiatives
Qualifications
- Bachelor's degree in Pharmacy
- Relevant clinical exposure, internship, or placement in a hospital and/or retail pharmacy setting
- Department of Health (DOH) license holder
- Knowledge of key pharmaceutical procedures and current best practices
- Good understanding of pharmacy related regulations
- Practical knowledge in Microsoft Office suite
