Job Overview

  • The Patient Coordinator is responsible for coordinating home healthcare services, managing patient referrals and appointments, and ensuring smooth communication between patients, families, healthcare professionals, and internal departments.
  • The role supports the delivery of high quality, patient centered care while ensuring compliance with company policies and regulatory requirements.

Key Responsibilities

  • Coordinate patient admissions, assessments, and scheduling of home healthcare services.
  • Serve as the primary point of contact for patients and their families regarding service inquiries and care coordination.
  • Maintain accurate patient records and ensure all required documentation is complete and up to date.
  • Schedule and coordinate visits for nurses, caregivers, and other healthcare professionals.
  • Communicate with physicians, hospitals, insurance providers, and other stakeholders to facilitate continuity of care.
  • Monitor patient service plans and coordinate any changes or follow up requirements.
  • Assist in obtaining and renewing necessary approvals, authorizations, and documentation.
  • Respond promptly to patient concerns and escalate issues when necessary.
  • Ensure compliance with company policies, regulatory requirements, and patient confidentiality standards.
  • Prepare reports and maintain service records as required by management.
  • Support quality improvement initiatives and contribute to patient satisfaction goals.
  • Perform other administrative and coordination duties as assigned.

Qualifications

  • UAE National.
  • Experience as Patient Coordinator.
  • Homecare or healthcare experience.
  • English and Arabic (Required).
