Reporting to the Manager, Primary Health Care, the Primary Health Care Nurse Practitioner (NP) is a registered nurse with extended class registration and advanced education. Working in partnership with other clinical and administrative staff in Primary Health Care to provide a full range of health care services to ongoing clients registered to the LAMP Primary Health Care program, the NP places clients at the center of their care, by providing them with knowledge and resources toward improved overall health and wellbeing.
Clients include the individual, families and the LAMP community.
Health care services emphasizes health promotion and disease prevention and the NP practices are evidence-based.
The Nurse Practitioner will ensure that all organizational policies are followed.
