NorthBay Integrative Primary Care (IPC) believes that lasting health is created through sustainable lifestyle change, meaningful patient partnerships, and whole-person care. The Integrative Primary Care RN serves as a trusted partner within the Integrative Primary Care Program, working in close collaboration with the Program MD’s and an on-site interdisciplinary team — including RNs, dietitians, and movement, stress management, and sleep specialists, and medical acupuncturists — to offer a highly personalized, relationship-based care experience built around a smaller patient panel. The Integrative Primary Care Program measures success not only by the clinical care delivered, but by patients’ ability to feel informed, supported, and engaged as active partners in their own health. While not a concierge medicine practice, the Integrative Primary Care participants do pay a program fee, not covered by insurance.

At NorthBay Health, the Program RN provides complex care management and individualized primary and preventive care. Guided by evidence-based Lifestyle Medicine principles, the role helps patients prevent, reverse, and manage chronic disease through sustainable behavior change. Within the integrative care model, the RN supports patients through individual coaching, shared medical appointments, group education, and coordination with the on-site interdisciplinary lifestyle and integrative medicine team, delivering an enhanced, concierge-like care experience. Working collaboratively with patients, providers, and the lifestyle medicine team, the RN serves as the direct liaison between the provider and patient, ensuring questions and concerns are addressed, directing annual lab and on site group learning engagement, coordinating access to NorthBay’s network of specialists, and empowering patient participants to achieve meaningful and sustainable lifestyle transformation. The ideal candidate personally embraces Lifestyle Medicine fundamentals and has a desire for continuous growth and development in the professional practice of nursing.

Primary Job Duties

The RN clinical duties include but are not limited to:

  • Collaborate as part of a 1:1 MD/RN care team within the Integrative Primary Care Program.
  • Manage complex care for an outpatient population with chronic health conditions.
  • Demonstrate a high level of service orientation in an outpatient program where patients pay a fee for access and participation.
  • Independently carry out clinical RN duties in an outpatient setting without the support of a medical assistant, phlebotomist, or administrative staff including: o Comprehensive patient assessment o Take vitals, maintains exam rooms o Phlebotomy-ordering/fulling/preparing personalized lab specimens o Counsel patients o Coordinates referrals and follow up o Schedules testing o Fulfills EHR documentation requirements o Documents in other portals as program components require o Maintaining patient correspondence and communication in a timely and efficient manner
  • Collaborates closely with physicians and the interdisciplinary care team to coordinate individualized patient care plans and reinforce lifestyle interventions.
  • Facilitates individual visits and educational group classes supporting lifestyle medicine principles.
  • Assists in developing educational materials, wellness resources, and patient engagement tools that reflect current evidence and NorthBay Health branding standards.
  • Supports lifestyle medicine, wellness, and community outreach initiatives that promote healthy living and disease prevention.
  • Participates in quality improvement initiatives, program development, and outcome measurement to continually enhance patient experience and clinical effectiveness.
  • Maintains current knowledge of emerging research in lifestyle medicine, integrative medicine and chronic disease prevention.
  • Demonstrates exceptional communication skills while building trusting, compassionate relationships with patients, families, providers, and colleagues.
  • Serves as an advocate for whole-person, patient-centered care that empowers individuals to become active participants in their health journey.
  • Adheres to policies, industry standards, best practices.
  • Promotes a quality, high touch, high service patient experience at all times.

Hours of Work: Based on business needs.

Compensation: $70.69 to $85.93 based on years of experience doing the duties of the role.
