At NorthBay Health, the Clinical Pharmacist II is responsible for providing the clinical judgment, professional knowledge and distribution skills in the Pharmacy Department to ensure that patients receive pharmaceutical care which is consistent with the goals of the Department and the Organization.
Primary Job Duties
- Reviews drug orders, patient profiles, and medical records to evaluate the appropriateness of drug therapy.
- Selects, compounds, prepares, and dispenses medications in accordance with departmental procedures and all applicable laws and regulations.
- Oversees and directs shift workflow and departmental activities performed by pharmacy technicians and interns.
- Consults with medical staff, nursing staff, patients, and other hospital personnel regarding all aspects of drug therapy and serves as a drug information resource.
- Maintains and ensures the accuracy and completeness of patient profiles in accordance with departmental procedures.
- Routinely performs pharmacy profile and nursing Medication Administration Record (MAR) comparisons.
- Documents all clinical interventions and formulary compliance/noncompliance activities in the Electronic Health Record (EHR).
- Identifies and reports adverse drug reactions (ADRs) and medication errors according to established departmental policies and procedures.
- Under approved medical staff protocols, may initiate and/or adjust medication dosages, administration intervals, routes of administration, or laboratory monitoring as appropriate.
- Assists with special projects and the collection of Quality Improvement (QI) data as assigned.
- Performs other duties as assigned.
