General Statement of Duties
- This is a hands-on and active supervisory position, which will oversee, support, and supervise a 24-hour group home for people with intellectual disabilities daily. Duties include development and management of the staff schedule; oversight of house maintenance needs; organization of staff responsibilities and monitoring completion; scheduling and support with resident medical appointments; taking a leadership role in the development of the resident's person-centered plan; planning for and assessing individuals to ensure an appropriate level of care is being delivered; development of a positive rapport with staff, families, medical personnel, and other providers; providing coverage as scheduled and in emergency situations; reviewing documentation, and being on-call for the assigned group home during the week and participating in the rotating weekend on-call schedule.
Essential Duties
- Exhibit and practice knowledge of Home & Community Based Services; implementation of Person-Centered Services and Planning.
- Training and supervision responsibilities for staff.
- Working knowledge of intellectual disability services and group home service provision; working knowledge of resources available to clients.
- Knowledge of the everyday needs and problems of the identified client group; the ability to establish and maintain effective working relationships with individuals with intellectual disabilities, co-workers, residents' families, the public, and other service providers; excellent organizational skills.
- Create and execute clear and concise program and service plans, as well as modify them as needed and appropriate; the ability to communicate clearly and effectively, both orally and in writing.
- Make independent judgments and follow best practices to ensure the safety and wellbeing of residents.
- Organize and ensure timely assessment in all aspects of an individual’s life. This includes coordination with outside service providers, day supports, etc. to assess risks, abilities, SIS, and medical needs.
- Meet monthly, quarterly, annual, and other documentation deadlines as needed. This will include client and program-specific documentation.
- Coordinate with supervisor for the hiring of Direct Support Professionals who will work best in the environment and meet the needs of individuals.
