Community Connection Worker

Child Welfare: Assist with identifying foster homes.

Mental Health & Addictions: Provide non-clinical support, advocacy, and referrals to the Health and Wellness program for counselling, addiction support and other wellness services.

Community Living: Help individuals with disabilities or developmental challenges access inclusive programs, housing, and independence support.

Shelters/Housing: provide resources for emergency shelters or transitional housing, addressing immediate needs and long-term planning.

Attend and support team meetings within the child and family services Dept.

Support individuals in navigating government systems and completing applications for income assistance, housing, medical services, etc.

Advocate for individuals facing systemic or service-based barriers.

Act as a liaison between community members and service providers.

Maintain accurate records of client interactions, service plans, and referrals.

Assist in collecting data to identify emerging needs and gaps in services.

Participate in case reviews, team meetings, and community planning sessions.

This role may include flexible hours including evenings or weekends as needed.

Must be comfortable with both office work and community-based outreach.

Physical activity such as home visits, transportation of clients, and event set-up may be required.
