Your Choice Program Direct Support Professional

Your Choice is a family-initiated program designed to bridge the gap in Homemaker/Personal Care (HPC) services that some individuals and families are experiencing through the Level 1, IO and SELF Waivers.

We are very excited to provide this service to individuals and families who are looking for a more self-directed program which is controlled by the individuals and their families.

WHAT MAKES THE YOUR CHOICE PROGRAM UNIQUE?

Your Choice is about self-determination.

It empowers people with disabilities to enhance the quality of their lives by providing them the opportunity for choice and control over their services and supports as well as acknowledging that they are the best judges of their own needs and how those needs should be met.

It also alleviates the family/individual from the burden of having to screen, verify, certify, and follow up to obtain records - all of which can seem overwhelming.

The result of this philosophy is a self-directed service that allows the individual, or their chosen family representative, to control the services they receive, recruit their support workers, as well as determine how and when services will be provided.

HOW DOES THE SUPPORT WORKER GET PAID?

Envision will support the individual, or their chosen family representative, by being the employer of record for their chosen support workers.

In addition, Envision will be responsible for certification, payroll taxes, workers’ compensation and other employer responsibilities.

The base rate of pay for a Your Choice DSP is $19 per hour with the option to earn an additional $1 hourly add-on for a qualifying behavioral add-on (if applicable) and $1 hourly add-on for DSP Longevity Program completion (if eligible).

Envision employees are paid bi-weekly.

HOW DO I GET STARTED?

Please submit your application to begin the process of applying to serve as a direct support professional for the individual/family that has chosen you to provide care.

Once received an Envision team member will reach out to you to schedule a meeting and can arrange for the remaining paperwork and screening to be completed.

Pay rate effective 1/1/24.
