Requirements
- Graduate of a school of professional nursing approved by ACEN or CCNE
- Current, full, active, and unrestricted registration as a graduate professional nurse in a US state or territory
- English Language Proficiency
- BSN and 5 years nursing experience preferred
- Outpatient Clinic/Office Setting experience preferred
- PACT Team experience preferred
- Phone Triage experience preferred
- Case Management experience preferred

Summary

Qualifications
- English Language Proficiency required.
- Graduate of a school of professional nursing approved by ACEN or CCNE.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a US state or territory.
- Grade Determinations based on education and experience (Nurse I, II, III).
- Preferred Experience: BSN, 5 years nursing experience, Outpatient Clinic/Office Setting, PACT Team, Phone Triage, Case Management.
Duties
- Primary Care Specialty Care Outpatient Staff Registered Nurse responsible for individual nursing practice.
- Provides patient care, care coordination, health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
- Formulates a plan of care based on nursing assessments and Veteran goals.
- Delegates elements of care to appropriate individuals.
