Success factors & skills required are
- Providing holistic aged care services through assessment, planning, implementation and evaluation of resident care support and services
- Provision of best practice clinical care
- Medication management, wound management, pain management, continence management, dementia care and palliative care
- Accurate and contemporaneous documentation
- Providing mentoring, guidance and support to the team
- Ability to work as part of a dedicated multidisciplinary team
- Liaising with external support services
