LPN / RN – Night Shift
Swanton Valley Rehabilitation and Healthcare Center Swanton, Ohio
Swanton Valley Rehabilitation and Healthcare Center is seeking a compassionate and dependable Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our nursing team on Night Shift .

Position Overview
As an LPN/RN at Swanton Valley, you will provide quality nursing care to our residents while helping maintain a safe, comfortable, and resident-centered environment. Our nurses play an important role in leading the care team and ensuring residents receive the attention and clinical support they deserve.
Responsibilities
- Provide direct nursing care in accordance with residents' plans of care
- Administer medications and treatments as ordered
- Complete nursing assessments and monitor changes in resident condition
- Communicate changes and concerns promptly to physicians and appropriate members of the care team
- Accurately complete required clinical documentation
- Supervise and support STNAs during the shift
- Assist with admissions, transfers, and discharges as needed
- Respond appropriately to changes in condition and emergency situations
