Role overview

Clinical care

• Assess and diagnose a wide range of primary healthcare conditions, order investigations, plan treatment, and make appropriate referrals.
• Consult specialists or consultants when admission may be required and observe patients in urgent care.
• Prescribe within applicable regulations, training, and registration requirements.
• Undertake screening procedures and telephone consultations as required.

Patient management

• Establish and maintain systems for managing patients with long-term conditions.
• Provide referrals, ongoing support, and coordination for patients, families, carers, and other services.
• Explain complex clinical information and provide advice that protects patient dignity and understanding.
• Promote optimum patient health through targeted health promotion activities.

Professional standards

• Maintain professional and ethical accountability within legal and ethical frameworks and hospital requirements.
• Follow DOH and JCI guidelines and standards.
• Maintain confidentiality, obtain informed consent, and protect the dignity and privacy of patients and families.
• Maintain clinical competence through current literature, professional development, and the hospital CME program.

Documentation and quality

• Create and maintain concise, contemporaneous computerized medical records and document care accurately and promptly.
• Record the reasons for investigations, analyze results, and document required actions in the medical record.
• Complete healthcare and practice administration documentation according to ICD and E/M guidelines.
• Use evidence-based practice to provide high-quality, clinically effective, and cost-effective primary care.

Collaboration and communication

• Collaborate with healthcare professionals and agencies and maintain effective links with primary and secondary care.
• Communicate appropriately with people from varied cultures and backgrounds, including people with disabilities and learning difficulties.
• Manage personal clinical responsibilities and act as a source of information for other clinical staff.
• Attend department and other scheduled meetings.
