Job Summary
- A General Practitioner (GP) provides comprehensive medical care to patients in their homes, ensuring safe, accessible, and patient centered healthcare services.
- The GP assesses, diagnoses, and manages patients’ medical conditions while coordinating with nurses, therapists, specialists, and the healthcare team to deliver quality care outside the traditional clinical setting.
Key Responsibilities
- Conduct home based patient consultations, medical assessments, and physical examinations
- Diagnose and manage acute and chronic medical conditions in a home healthcare setting
- Develop individualized treatment plans based on patient needs and medical history
- Perform follow up visits and monitor patient progress and recovery
- Review laboratory results, diagnostic reports, and medical records
- Prescribe medications and provide appropriate medical interventions
- Manage elderly patients, post operative cases, chronic disease patients, and patients requiring ongoing medical support
- Coordinate with nurses, physiotherapists, caregivers, and specialists to ensure continuity of care
- Provide health education and guidance to patients and family members regarding disease management, medication compliance, and preventive care
- Identify changes in patient conditions and escalate cases requiring urgent medical attention
- Maintain accurate and timely documentation of patient assessments, treatment plans, and progress notes
