Responsibilities
- Code medical diagnoses, procedures, and services using standardized classification systems for all encounter types
- Perform concurrent coding by reviewing medical records in real time during hospitalization
- Determine appropriate Diagnosis Related Group (DRG) for inpatient cases
- Ensure charge capture by validating and reconciling charges against clinical documentation
- Collaborate with departments to resolve coding and billing issues
Requirements
- Expert level knowledge of clinical coding systems including ICD 10 CM, CPT 4, HCPCS, and LOINC
- Strong understanding of human anatomy and physiology
- Ability to code all encounter types: outpatient, inpatient, emergency, daycare
- Experience with concurrent coding and DRG assignment
- Collaboration skills with Quality, Finance, Revenue Cycle, and other departments

Job Summary
- A Clinical Coder is a skilled medical professional responsible for accurately coding medical diagnoses, procedures, and services.
- Requires expert level knowledge of clinical coding systems including ICD 10 CM, CPT 4, HCPCS, and LOINC.
- Must have strong understanding of human anatomy and physiology to interpret medical classifications.
Key Responsibilities
- Code all diagnostic, laboratory, radiology, operative, surgical, office procedures, E/M services, and consultation codes for all encounter types.
- Independently perform quality checks on own work to ensure accuracy and compliance.
- Perform concurrent coding by reviewing medical records in real time during hospitalization.
- Determine appropriate DRG for inpatient cases to optimize reimbursement ethically.
- Review and analyze medical records to extract relevant data and ensure precise code assignment.
- Ensure charge capture by validating and reconciling charges against clinical documentation.
- Collaborate with Quality, Finance, Revenue Cycle, Registration, Scheduling, Case Management, and Compliance departments.
