Case Manager Lead

• This position does not have formal hiring or termination authority.
• Provides coaching, technical guidance, case review, and performance feedback to HRSN case management staff in coordination with the Program Supervisor or Program Manager.

• High school diploma or equivalent required.
• Minimum of 1–3 years of experience in social services, housing navigation, or case management.
• Demonstrated ability to interpret and apply federal, state, and local program requirements.
• Strong organizational, communication, and problem-solving skills.
• Experience maintaining confidential client records and using data management systems.
• Basic computer proficiency (Google Suite, case management software).
• Ability to collaborate with multidisciplinary teams and community partners.
• Valid driver’s license and reliable transportation required.

• Four (4) or more years of experience in case management, HRSN, or care coordination.
• Prior leadership or mentoring experience supporting case management staff.
• Familiarity with Oregon Health Plan, CCOs, and OHA 1115 Waiver HRSN programs.
• Knowledge of trauma-informed care, motivational interviewing, and person-centered planning.
• Experience working with diverse, marginalized, or rural populations.
• Bilingual proficiency preferred.
• Experience using systems such as Unite Us, Epic, or Salesforce Health Cloud.

• Ability to sit for extended periods while reviewing case documentation and completing reports.
• Ability to walk up and down stairs and move throughout shelter, office, and community sites.
• Ability to lift up to 20 pounds and assist with light physical tasks (e.g., transporting materials or setting up meeting spaces).
• Ability to perform job duties that may involve bending, twisting, or reaching.
• Ability to travel locally for client support, court accompaniment, meetings, and community partner coordination.
• Fully on-site, with potential for hybrid flexibility after the first 90 days of employment.
