Case Manager

Demonstrate excellent customer service consistent with the standards contained in the Vision, Mission, and Values of the organization.

Manage a caseload and ensure positive outcomes through the timely coordination of all case management services with a warm and welcoming attitude; Meet contract standards and provide linkages to substance abuse or mental health treatment as needed, and transportation to other involved or needed providers to coordinate member’s care and housing solutions.

Provide supportive counseling and participate in crisis intervention services with individuals including mental status and risk assessment and linkage to crisis teams and hospitals; coordinate written crisis plans when indicated in coordination with the client's psychiatric provider.

Timely and accurately document clinical records in the required systems of record and maintain records of referrals, assessments and placement plans in compliance with IHS standards of documentation and all contract requirements.

Make collaborative outreach attempts, including transportation of clients to other involved or needed providers to coordinate member’s care as necessary.
