Peter’s Hospital Site of HHS is a Complex Care Hospital (Chronic Care).

There are four inpatient specialty programs ranging is size from 44 beds to 63 beds, as follows: Behavioural Health, Medically Complex, Palliative Care and Restorative Care.

There are also specialty outpatient clinics geared to the senior population offered through St.

Peter’s Centre for Healthy Aging.

All inpatient programs provide a full, on-site Interdisciplinary team, model of care to address the assessment, treatment and care needs of their complex populations.

All programs aim to relieve suffering from significant illnesses with complicated symptoms, distress, loss of function and/or safety and behavior challenges, while promoting optimal functioning, enhanced quality of life and recovery, wherever possible.

The potential for discharge planning to home or an alternate level of care is routinely assessed.

Our teams strive for patient/family driven decision making and foster clinical excellence through the support of ongoing education and research activities.

Days (8 hour) Weekends and Holidays, as well as weekdays

• Graduate of an approved School of Nursing and currently registered with the CNO; BScN. preferred
• Demonstrated experience in effective case management
• Demonstrated skills in program development
• Demonstrated motivation to evaluate quality and cost
• Demonstrated strong assessment skills for patients and families with complex needs
• Demonstrated ability to coordinate the care of patients with complex needs with other team members (including family) and community agencies
• Demonstrated ability to work effectively with a variety of health care professionals onsite & in the community
• Demonstrated commitment to providing care that is based on documented best practice
• Demonstrated leadership, decision making abilities and able to take acceptable risks
• Demonstrated ability to adjust quickly to changing conditions - both patient and environmental conditions
• Demonstrated responsiveness to challenges and ability to work well under pressure
• Demonstrated commitment to teaching and life-long learning
• Demonstrated commitment to teaching and preceptorship of nursing students helping them to develop the nursing leadership skills
• Ability to attend work regularly as demonstrated by an acceptable attendance record
