• Develop respectful and trusting relationships with clients using a trauma informed practice and Psychosocial Rehabilitation principles
• Assist clients in creating individualized plans tailored to their needs and designed to improve quality of life and reflect accountability and recovery
• Support clients in identifying and referring to resources specific to their needs based on assessment
• Coordinate care and act as a liaison by connecting clients with social, medical, and community support.
• Provide crisis intervention and help develop crisis plans.
• Represent the program professionally with clients, families, and community partners.
• Mediate conflicts and help clients learn problem-solving skills.
• Organize or support social activities to reduce isolation.
• Maintain accurate client records and documentation.
• Work collaboratively with the team, contribute ideas, and support new staff or volunteers.
• Provide case management and connect clients with social, health and community services like housing and ID support.
• Plan and facilitate group activities/ events to promote engagement/ participation, development of social networks and to help build skills
• Help clients access financial supports such as Ontario Works (OW) and Ontario Disability Support Program (ODSP) or other financial resources as required.
• Other duties as assigned by the DTE JC Crown Lead in collaboration with the Program Manager and/or designated supervisor.
• Post-secondary education in Social Services / Human Services or equivalent field.
• Minimum 3 years’ working experience with client population group.
