Responsibilities
- Assist the Registered Nurse in planning, implementing and evaluating patient care
- Monitor the impact of nursing care and maintain ongoing communication with the Registered Nurse
- Provide education to the family regarding aspects of care as appropriate
- Assist physicians/therapists with procedures
- Maintain patient confidentiality at all times
- Complete annual education requirements and maintain regulatory requirements
- Participate in Performance Improvement and Continuous Quality Improvement activities
Requirements
- Experience as a nursing assistant or similar role in homecare or clinical setting
- Familiarity with DOH Severe Care Domains
- Ability to assist physicians/therapists with procedures
- Strong organizational and communication skills
- Commitment to maintaining clinical knowledge and ongoing education

Duties
- Assists the Registered Nurse in planning, implementing and evaluating patient care.
- Is familiar with, and understands, the Severe Care Domains of DOH.
- Monitors the impact of nursing care and maintains ongoing communication with the Registered Nurse.
- Demonstrates an understanding and utilization of Evidenced Based Healthcare.
- Advocates in the best interest of the patient and family at all times.
- Provides education to the family regarding aspects of care as appropriate.
- Demonstrates an ability to assist physicians/therapists with procedures.
- Demonstrates the ability to be flexible, organized and function under stressful situations.
- Adheres to dress code; appearance is professional, neat and clean.
- Maintains patient confidentiality at all times.
- Reports to work on time and as scheduled, completes work within designated time.
- Completes annual education requirements and maintains regulatory requirements.
Qualifications
- Experience as a nursing assistant or similar role in homecare or clinical setting.
- Familiarity with DOH Severe Care Domains.
- Ability to assist physicians/therapists with procedures.
- Strong organizational and communication skills.
- Commitment to maintaining clinical knowledge and ongoing education.
