YMCA ReConnect Alternative Dispatch and Yard Outreach

Support members with basic needs, weather response, and wellness checks.

Lead Hub Yard operations, including access to storage bins and maintaining a safe, respectful environment.

Contribute to yard cleanliness and light maintenance to support a welcoming space.

Case Management and Resource Navigation Provide goal-focused, case management-style support during designated shifts or as appropriate.

Support members in identifying priorities and working toward housing stability and overall well-being.

Assist with system navigation; including housing applications, income supports, identification, and referrals.

Advocate for members and help reduce barriers to accessing services.

Coordination and Safety Communicate clearly with GardaWorld security, supervisors, and team members to support coordinated service delivery.

Notify appropriate staff when responding to dispatch calls to ensure continued coverage within the Hub Yard.

Work collaboratively with security, leading member interactions while engaging support when situations escalate.

Maintain situational awareness and prioritize safety for members, staff, and partners at all times.

Programming and Engagement Plan and deliver low-barrier programming within the Hub Yard, such as gardening, life skills, and recreational or creative activities.

Foster a sense of community and belonging through inclusive, member-driven engagement opportunities.

Data Management and Reporting: Utilize HIFIS to maintain accurate, timely, and complete client records, including intake forms, service notes, assessments, and case plans.

Track referrals, outcomes, and follow-ups in accordance with program standards and funder expectations.

Ensure secure storage, logging, and handling of sensitive documents such as ID, mail, and tax-related paperwork.

Uphold strict standards of privacy, confidentiality, and data protection in all case documentation and information sharing.

Contribute to program monitoring and evaluation through accurate data entry and participation in quarterly and annual reporting processes.
