The Targeted Case Manager is responsible to assist clients and families in accessing behavioral health treatment, medical interventions, educational, social, development, and other support services.
Targeted Case Manager advocates for the needs of the clients and families and works closely with our Outpatient clinical team and community partners to ensure the client’s needs are being met across settings.
Targeted Case Managers focus on linking clients to local resources and enhancing social and emotional functioning and life skills.
