Social Worker – Full Time Neffsville Nursing & Rehab | Lancaster, PA

Key Responsibilities

• Complete social assessments and care plans addressing residents’ psychosocial needs with realistic, actionable goals
• Participate actively in pre-admission and admission processes
• Provide compassionate support to residents and families during adjustment, crisis, or periods of heightened need
• Document behavioral and psychosocial updates in accordance with facility and regulatory requirements
• Lead individual and group support as needed
• Develop and grow volunteer programs tailored to resident needs
• Orient new residents and families to the center to ensure a smooth transition

Qualifications

• Bachelor’s degree in Social Work or related field preferred
• Knowledge of long-term care social services practices, regulations, and guidelines
• Strong communication, empathy, and independent problem-solving skills
