Social Services Director

Qualifications:

• Bachelor's degree in Social Work, Human Services, Psychology, Sociology, or related field
• Previous experience in a Skilled Nursing Facility or healthcare setting
• Knowledge of resident rights, care planning, and discharge planning
• Strong communication, organizational, and interpersonal skills

Responsibilities:

• Complete psychosocial assessments and care plans
• Coordinate resident discharge and transitional care services
• Provide support and resources to residents and families
• Participate in care conferences and interdisciplinary team meetings
• Ensure compliance with federal and state regulations
• Advocate for resident well-being and quality of life
