Complete initial assessments with patients referred to and eligible for the social prescribing service. Assessing their current wellbeing and identification of what matters to them.
Manage a caseload of social prescribing patients providing follow-up support and ensuring they are able to access ‘prescribed’ services with confidence.
Be responsible for the scheduling and sequencing of patients social prescribing activities
Liaise with Health and Prison departments, to develop awareness and support delivery of the service. Ensure effective cross-departmental working in order to meet our aims and objectives of a whole system and asset-based approach to social prescribing.
Ensure services are consistent with our stated values.
