ESSENTIAL FUNCTIONS/DUTIES
- Demonstrates the ability to function as a Registered Nurse
- Telephonically triages patients and navigates care based upon evidence-based protocols.
- Patient education across the health care continuum.
- Telemonitoring activities engaging healthy patient behaviors.
