- Coordinate developmental assessment appointments and therapy services
- Act as the primary liaison between families, healthcare providers, therapists, schools, and community agencies
- Conduct intake interviews and gather relevant medical, developmental, and social information
- Develop, implement, and monitor individualized service plans
- Schedule appointments and maintain service calendars
- Ensure timely referrals to specialists and community support programs
- Maintain accurate and confidential client records and documentation
- Monitor service delivery and follow up on missed appointments or unmet needs
- Educate families about available services, benefits, and community resources
- Assist families with accessing financial assistance or government support programs when applicable
- Participate in multidisciplinary team meetings and case conferences
- Prepare reports, statistics, and documentation for quality assurance and compliance

- Experience in case management, healthcare coordination, or related field
- Knowledge of developmental delays, disabilities, or special needs services
- Strong organizational and communication skills
