Carefirst Seniors & Community Services Association is a non-profit charitable community services agency established since 1976.
Our spectrum of services has grown from the delivery of Chinese meals-on-wheels service to a full range of community support services, covering the whole Greater Toronto Area.
With a staff of 550 members (including full-time and part-time staff) and the help of a pool of 850 active volunteers, we serve over 15,000 clients a year, including seniors, adults, youth with physical, social, emotional and health challenges. 3,000 of whom are "home-bound" and frail.
The Carefirst Transitional Care Centre is a 30-bed facility providing short stay respite care service, aiming to create an environment promoting exceptional recovery and simultaneously enriching the quality of life for individuals who are in need.

We are currently seeking Registered Practical Nurses (RPNs) to fill a vacant position at our Transitional Care Centre.
Term: Casual
Shift : Overnight shift (11pm – 7am) on Tuesday and Saturday plus other nights on weekdays, weekend & holidays if required
Pay Range : $25.64/hour - $32.31/hour
Responsibilities
- Provide personalized nursing care to patients in 24-hr respite care setting, including administer medication, dressing changes and monitor patients’ conditions in order to provide appropriate and timely interventions to reduce risk and enhance health outcome
- Provide ongoing case management and care coordination
- Observe and document patient conditions and changes
- Provide direction, support, and delegation to PSWs, as appropriate
- Perform shift supervision and shift work, including overnights
Requirements
- Diploma in Nursing, and a current member of the College of Nurses of Ontario in good standing
- 1-2 years’ experience in a hospital/long-term care facility
- Current basic cardiac life support (BLS) certification
- Outstanding clinical skills with good judgment and ability to meet patients’ needs
- Strong understanding of nursing functions, chronic disease management, and geriatric care
- Ability to work independently with little direct supervision
- Excellent customer service with strong interpersonal, communication, and problem solving skills
- Proficient computer skills in Microsoft Office and EMR (Telus Practice Solutions)
- Flexible and adaptable to change in work assignments and schedules (shift work, overnight, weekends, and statutory holidays)
- Proficient in English, and ability to speak a second language that reflects the community served, eg. Cantonese and/or Mandarin, is an asset
- A valid vulnerable sector police record check completed within one year
