Registered Nurse (RN)

Complete skilled nursing visits in patients' homes

Perform comprehensive nursing assessments

Complete Start of Care, Recertification, Resumption of Care, and Discharge visits as assigned

Develop and update individualized plans of care

Provide wound care, medication management, and disease education

Monitor patients for changes in condition

Educate patients and caregivers

Communicate concerns and changes promptly to physicians and the clinical team

Coordinate care with therapists and other disciplines

Complete accurate and timely documentation using WellSky

Active Virginia Registered Nurse (RN) License Required

Graduate of an accredited nursing program

Current CPR/BLS Certification

Valid Driver's License and reliable transportation

Strong clinical assessment and communication skills

Ability to work independently in patients' homes

Home Health and OASIS Experience Preferred, but Not Required
