Physical Therapist (PT)-Warrenton, Culpeper

Complete physical therapy evaluations in patients' homes

Develop individualized plans of care

Provide therapeutic exercise, gait, balance, transfer, and mobility training

Help patients improve strength and functional independence

Assess home safety and fall risks

Educate patients and caregivers

Monitor progress and adjust treatment plans as needed

Coordinate care with physicians, nurses, therapists, and other team members

Complete accurate and timely documentation using WellSky

Participate in reassessments and discharge planning

Active Virginia Physical Therapist License Required

Graduate of an accredited Physical Therapy program

Valid Driver's License and reliable transportation

Strong clinical assessment and communication skills

Ability to work independently in patients' homes

Home Health Experience Preferred, but Not Required
