Responsibilities
- Prepare clinical and progress notes, coordinate services, and act as liaison between physician and family.
- Monitor and record vital signs, report abnormal values to treating physicians.
- Administer prescribed medications and monitor intravenous infusions.
- Provide care for patients with tracheostomy, gastrostomy, colostomy, pacemaker, peritoneal dialysis, and ventilator.
- Deliver nursing care to patients with hemiplegia, paraplegia, Bell's palsy, cerebral palsy, terminally ill, and elderly.
- Care for newborns, pre and post partum mothers, and physically challenged patients at home or educational facilities.
- Assist with simple exercises, ambulation, and accompany patients to social gatherings and medical appointments.
- Provide wound care to prevent pressure ulcers.
- Report and record medical events, procedures, and patient condition to supervisor or physician.
Requirements
- Valid nursing license
- Experience in homecare or clinical nursing
- Ability to monitor and record vital signs
- Competence in administering medications and IV therapies
- Knowledge of tracheostomy, gastrostomy, and colostomy care

Principal Accountabilities
- Prepares clinical and progress notes, coordinates services, works as liaison between treating physician and other personnel of changes in the client's condition and family members.
- Counsels the clients and family in meeting nursing and related needs, participates in in service programs, and supervises and teaches other nursing personnel.
Specific Duties
- Monitoring and recording vital signs and reporting to the treating physicians for the abnormal values.
- Administering prescribed medications.
- Monitoring intravenous infusions and parenteral therapies.
- Caring of clients with Tracheostomy, gastrostomy and colostomy.
- Carrying out nursing care to clients with Pace maker, peritoneal dialysis, and clients on ventilator.
- Delivering Nursing care of clients with hemiplegic, paraplegic, Bells palsy, cerebral palsy, terminally ill, and elderly etc.
- Caring new born, pre and post partum mothers.
- Caring of clients with physically challenged and those who need special care at home or at their educational facility.
- Delivering simple exercises, helping in ambulation and accompanying clients to social gathering, hospitals, other treatment facilities and out doors.
- Caring for the wound of the patient in order to prevent exacerbation of pressure ulcers.
- Reporting and recording of medical events, procedures and other medical parameters.
- Maintain records of patient care, condition, progress, and problems in order to report and discuss observations with a supervisor or case manager, or Medical Doctor.
