Position Summary
The Project Manager will support the planning, implementation, and ongoing management of the Community Health Worker (CHW) Pilot, a state-approved and funded initiative under the NJ FamilyCare program. Position Responsibilities
- Lead pilot implementation across all phases, including planning, execution, monitoring, and evaluation.
- Develop and manage project plans, timelines, and key milestones aligned to state requirements and program design.
- Coordinate cross-functional teams including care management, behavioral health, pharmacy, providers, community partners, and external stakeholders.
- Ensure alignment of staffing, workflows, training, and operational readiness for pilot launch and execution.
