Program Manager (Team Coordinator) – Supported Living

Your Responsibilities

• You will oversee community-based and daily support services for individuals living in their own homes in South King County. Key responsibilities include:
• Leadership: Provide direct supervision to up to 20 Direct Support Professionals (DSPs), mentor staff, and conduct performance reviews to support a high-performing team.
• Advocacy: Coordinate and document health care services for individuals in accordance with state and agency requirements.
• Support Management: Monitor and maintain each individual’s medical and service benefits to support continued access to essential resources.
• Direct Support: Provide immediate assistance during emergencies, including physical transfers and personal hygiene support.
• Financial Oversight: Reconcile each individual’s financial records monthly to ensure accuracy and accountability.
• Collaboration: Schedule and attend required monthly meetings, prepare written meeting documentation, and promote open communication and teamwork.

Qualifications/Experience

• Preferred experience includes at least three years in a supervisory role and five years providing supported living services in community settings. Strong candidates will also demonstrate: The ability to build supportive relationships with DSPs, individuals receiving services, and their families. Experience developing individualized support plans tailored to each person’s needs. The flexibility to work varying schedules as required. Excellent listening and communication skills, along with strong organizational and documentation capabilities. A deep understanding of person-centered support philosophies.
