In collaboration with direct supervisor, and Senior Leaders of Clinical and Operations, defines and advances the strategic direction for Clinical & Nursing services. This includes scope, programs, care delivery models and staffing. Implements and maintains a comprehensive nursing and care management model focused on whole-person care within an integrated primary care team.
Accountable for the development and maintenance of nursing content including associated nursing practice compliance, evidence-based nursing standards, nurse care management practices, workflows, protocols, procedures, patient safety guidelines, scope of practice, and other quality-related and regulatory considerations.
In close collaboration with direct supervisors, Clinical and Operational leaders, advances the care model, interdisciplinary team integration and high-quality patient care. Supports nurse supervisors to manage daily operations, including appropriate staffing levels and patient care.
Develop and maintain learning and training programs and support for nurses including professional development, onboarding, orientation and specific skill development or reinforcement needs for the nursing team.
Provide clinical oversight and evaluation of various screening tools and nursing interventions such as: Social Drivers of Health screening, KATZ, GAD7, PHQ9, motivational interviewing, care management assessments, and safety planning with patients. Supplements the learning and expertise of all direct reports including staff and leadership within discipline and implementing changes when indicated. Provides learnings and facilitates external learnings as needed.
Collaborate with clinic Nurse Supervisors to deliver successful hiring, performance management, talent development, employee relations, recognition and training of diverse staff by providing effective leadership. Ensure stability and reliability of nursing teams across HealthPoint, including maintaining required licensure, learnings and effective recruitment and retention strategies. Provide direct supervision of the functions above for Care Support and Community nurses.
Provide clinical input and/or oversight as appropriate for practices or workflows such as point of care testing, labs, infection control, standing orders, quality improvement initiatives, and standard work. Collaborate with Director of Operations Programs and Patient Experience to align MA/DA SME practices and onboarding for these practices and workflows.
Continuously evaluate and assess nursing program strengths and opportunities. Provide recommendations and solutions to complex programmatic, contractual, and funding stewardship situations in collaboration with Supervisor and Senior Directors of Clinical and Operations.
Identify barriers and potential solutions to program and staff development across sites. Advocate for programmatic and team integration and sustainability, including effective recruitment and retention strategies, as well as opportunities for standardization and collaboration.
Articulate and integrate the Nursing roles into the care team to provide clinical insights and direction from a nursing perspective at multiple levels of the organization.
Partner with internal departments to streamline and optimize internal processes to continually improve workflows, system functionality, personnel oversight strategies, electronic resources, communication, and administrative functions.
Build and strengthen community partnerships to advance health equity, reduce barriers to care and/or enhance access and services for patients receiving nursing services, acting as liaison and ambassador for nursing or HealthPoint as needed.
Continuously align work priorities with organizational goals, adjusting strategies and programs accordingly.
Adeptly navigate sensitive conversations with staff and teams including development planning, coaching, work performance and productivity using emotional intelligence that nurtures self awareness and growth in team members. Ensure effective communication between staff, care team members, coworkers, patients, and support systems.
Collect, analyze, and present clinical, operational and/or financial data regarding the performance of designated Nursing teams or programs to inform and expand services as needed.
Generate and maintain regular reports and utilize electronic resources to meet program, budget, and contractual requirements. Present data and information to internal or external groups as needed.

This position has supervisory responsibilities. Direct supervision for Care Support and Community Nurses. Matrix Supervisory relationship with Operations leaders that have nursing staff, Matrix input to clinical support SMEs under the Director of Patient Experience and Operations Programs.
