Role Overview

The medical coding and clinical documentation improvement specialist supports an outpatient medical center.

The role focuses on documentation review, coding accuracy, insurance requirements, prior authorizations, and compliant billing support.

Key Responsibilities

  • Review outpatient notes, diagnoses, procedures, investigations, and treatment plans for completeness and accuracy.
  • Identify documentation gaps, inconsistencies, missing diagnoses, incomplete assessments, and unclear physician notes.
  • Communicate with physicians and clinical staff to clarify documentation and complete medical records.
  • Issue compliant clinical documentation queries when required.
  • Assign accurate ICD-10, CPT, and other applicable coding classifications.
  • Verify medical necessity and documentation support for coding and billing.
  • Review insurance eligibility, coverage limitations, exclusions, and authorization requirements.
  • Prepare prior authorization requests and follow up with insurers on pending approvals or information requests.
  • Ensure records are complete and compliant before claim submission and billing.

Required Qualifications

  • A bachelor's degree or diploma in a healthcare-related field such as Health Information Management, Nursing, Pharmacy, Medical Laboratory Sciences, or Physiotherapy is required.
  • The role requires 2–5 years of experience in medical coding, insurance approvals, clinical documentation improvement, or revenue cycle management.
  • Strong knowledge of ICD-10 coding and outpatient documentation requirements is required.
  • Experience with insurance pre-authorizations and claim requirements is required.
  • CPC, CCS, COC, CIC, or an equivalent certification is preferred.

Schedule

  • The posting states duty hours of 10 hours daily with one day off.
