Responsibilities:
- Supervise and support direct care staff, including Med Techs and Resident Care Aides.
- Monitor resident health status and ensure timely follow-up on changes in condition.
- Ensure resident charts are established upon admission and maintained in compliance with Department of Health regulations.
- Update and implement medication orders in accordance with physician directives.
- Ensure laboratory requisitions are completed, processed, and followed up on appropriately.
- Assist with physician and podiatry visits and coordinate follow-up care.
- Conduct chart audits and monitor documentation for accuracy and compliance.
- Maintain accurate resident census records and communicate anticipated discharges to the Administrator.
- Ensure new hires receive appropriate training and maintain training documentation.
- Conduct staff interviews and assist with hiring decisions.
- Maintain and update medical equipment inventory records.
- Participate in the on-call rotation as assigned.
- Promote a positive, resident-centered environment that supports dignity, independence, and quality of life.
Qualifications:
- Current New York State LPN license in good standing.
- Previous nursing supervisory experience in assisted living, long-term care, memory care, or a related healthcare setting preferred.
- Strong leadership, communication, and organizational skills.
- Knowledge of New York State Assisted Living regulations and documentation requirements.
- Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
