About the Role
As a RN/LPN, you will be responsible for providing direct patient care, assessing patient needs, and implementing individualized care plans. You will work closely with our interdisciplinary team to ensure that our patients receive comprehensive care that meets their unique needs. Responsibilities
- Provide direct patient care, including administering medications, wound care, and other treatments as directed • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team • Monitor and report patient progress, making adjustments to care plans as needed • Develop and maintain accurate and up-to-date patient records • Collaborate with other healthcare professionals to ensure comprehensive care • Participate in quality improvement initiatives and professional development activities
