As an Intake Worker, you will play a central role in connecting children and families to services across the Kenora and Rainy River districts.
You will lead a family-centred intake process, gather information using screening tools, and work collaboratively with families to identify needs, strengths, and service options .
Reporting to the Clinical Manager, you will:
• Answer the centralized intake phone line, receive referrals, and monitor intake email.
• Facilitate referrals through multiple pathways and ensure smooth access to services.
• Provide information to families and community partners about available programs and supports.
• Conduct a family-centred intake process, including informed consent and screening tools.
• Support families in identifying goals and selecting services that meet their needs.
• Process referrals and prepare documentation for service coordination and follow-up.
• Identify interim supports for families while they wait for services.
• Maintain current knowledge of local and regional services across Kenora and Rainy River districts.
• Participate in meetings, training, and small projects as required.
• Perform other duties as required.
