A Day in the Life:

• Perform comprehensive nursing assessments of patients and families to identify physical, psychosocial, and emotional needs
• Develop, implement, and evaluate individualized, patient-centered care plans focused on comfort, symptom control, and quality of life
• Provide skilled nursing interventions, including pain and symptom management, medication administration, wound care, and ongoing clinical monitoring
• Utilize critical thinking and clinical judgment to recognize changes in condition and intervene appropriately
• Serve as the primary RN case manager, coordinating care across the interdisciplinary team
• Collaborate closely with physicians, nurse practitioners, social workers, chaplains, and hospice aides to ensure seamless care delivery
• Facilitate communication among team members to support timely and effective care planning
• Maintain accurate, timely, and thorough nursing documentation in accordance with regulatory and organizational standards (Wellsky/Consolo)
• Participate actively in interdisciplinary team (IDT) meetings, care conferences, and ongoing care planning

• Current Registered Nurse license in the state which you will be working in.
• One (1) years experience as a RN Case Manager or similar role working for a hospice agency is preferred.
• Must have a current Driver's license and dependable transportation.
