- Develops various channels of referrals through the selection, acquisition, retention, and growth of business partnerships with referral sources.
- Consistently visits communicates, and monitors provider, community, customer, payer, and patient perceptions of Concho Hearts Hospice as a high-quality provider of services.
- Conduct educational in-services and community outreach to promote our superior hospice care services.
- Educates patients and their families about hospice expectations.
- Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to a home.
- Operation / Compliance standards for patient care.
